Identity Theft Victim Dispute Letter (FTC Report Attached)

Subject: Formal Identity Theft Dispute – Immediate Action Required Under FCRA 15 U.S.C. § 1681c-2

To Whom It May Concern,

I am a victim of identity theft, and fraudulent accounts have been illegally placed on my credit file. This is a formal demand for the immediate removal of all fraudulent accounts, inquiries, and transactions under FCRA 15 U.S.C. § 1681c-2, which mandates that credit reporting agencies must block fraudulent information when provided with proper notice.

The following accounts and inquiries were fraudulently opened in my name:

Account NameAccount NumberDate OpenedReported Balance

Enclosed, you will find the Federal Trade Commission (FTC) Identity Theft Report I have filed, along with a police report documenting this crime.

Your Legal Responsibilities:

Under FCRA 15 U.S.C. § 1681c-2, you must:

  • Block and permanently delete all fraudulent accounts and inquiries within 4 business days.
  • Cease all collection activity on fraudulent accounts
  • Provide me with a written confirmation of the deletion and an updated credit report.

Failure to comply will result in formal complaints to the CFPB, FTC, my State Attorney General, and legal action for willful noncompliance. If necessary, I will also seek damages under FCRA 15 U.S.C. § 1681n, which provides statutory and actual damages, punitive damages, and attorney fees.

Sincerely,
